If you are interested in evoking the right of first refusal on behalf of YOURSELF we will need the below information a week prior to the pet becoming available.

All requests will need to be approved by the AZ CARE board of directors.

    3. Name of adopter (If other than foster parent)
    4. Email Address of adopter(required)
    5. Cell number of adopter
    6. Street Address of adopter

    9. Zip Code of adopter
    10. Pet interested in
    11. Name of the pet you are evoking the right of first refusal for
    1. By filling out this form you acknowledge that all Right of First Refusal requests must be reviewed by the board of directors and must be submitted at least one week in advance prior to the pets availability date.
